The team from the CRE in Telehealth is delighted to invite you to another forum of its monthly series. Please join us for a presentation on Brisbane South PHN supporting services to deliver the right care at the right place at the right time, by Luke Maloney, Business Development Manager and Tracey De Angelis, General Manager Commissioning, Brisbane South Primary Health Network. Tracey and Luke will be presenting on the role of Primary Health Networks as commissioning entities, the move towards outcome based commissioning and how digital health is a critical enabler in the march towards patient centred medical homes.
